Notice to Close


For the purposes of Staff training this surgery, along with others in the Thurrock area, will be closed on the afternoon of  Tuesday 12th March 2024.  Please note we shall be closed from 12.30pm.
During closure an Emergency Only service will be in operation and you are asked to telephone 01375 480000 where a contact number will be provided.
The surgery will open as normal on Wednesday 13th March 2024

Prescriptions


Please allow THREE working days for repeat prescriptions to be processed, allow an extra day if your request has been sent after 11.30am.  Please bear this in mind when requesting your prescription, including if requesting via a pharmacy.  Thank you.

New Patient Registration

registrationTo register you will need to be living in Chafford Hundred (within our boundary area) and to produce your medical card, together with your birth certificate/passport plus a utility bill (dated within the last 3 months).  Please provide copies as we are unable to return original documents.  In exceptional circumstances, we may be able to register you without production of a medical card but you will need to know your National Health Number.  You will need to visit the surgery with your identification documents, and you will be booked a future appointment for a New Patient Health Check which are held on Wednesdays.

Mornings             Monday - Friday          9.00am - 12.00pm
AfternoonsMonday - Wednesday & Friday2.00pm - 5.00pm

You will be registered with the group practice but you may wish to request to have a “preferred” GP of your choice.  Please feel free to arrange this with the receptionist at your registration.

At your registration appointment, the healthcare assistant will discuss your medical history and undertake a BP and weight check.  She will ask you to produce a urine sample (if you have not brought one with you, a bottle can be obtained from reception prior to your appointment).

If you are taking any regular medication, including contraception, you should bring the packets with you for the nurse to record.  Please ensure that you book a routine appointment with the doctor to obtain your first prescription from this surgery.

Children under the age of five are not required to attend a new patient registration appointment.  However, we will require some important information, such as immunisation history, and the receptionists will give you a form to complete to this effect.
