IMPORTANT MESSAGE RE: CORONAVIRUS

AS A PRECAUTION ONLY, the Practice has taken the decision to ask ALL patients NOT to attend the Surgery at any time.

All appointments will now move to telephone triage, if necessary the Doctor will arrange an appointment for you to be seen.

If you already have an appointment booked these will also move to telephone triage ONLY.

To book a telephone triage appointment, arrange for your prescription to be delivered to a pharmacy or any other query please contact Reception on 01375 480000

DUE TO THE CURRENT CORONAVIRUS SITUATION, ALL APPOINTMENTS BOOKED VIA OUR ONLINE  FACILITY ARE TRIAGE APPOINTMENTS AS SUCH YOU WILL RECEIVE A TELEPHONE CALL FROM THE DOCTOR AT YOUR ALLOTTED APPOINTMENT TIME.  THIS IS DUE TO IT BEING NECESSARY TO DETERMINE WHETHER PATIENTS CAN ATTEND THE SURGERY OR SHOULD BE CONTACTING NHS 111.

APPOINTMENTS AT OTHER LOCATIONS

Extra GP and Nurse appointments are available at a number of Hubs within Thurrock.  Hubs are open 7pm-9pm every weekday and 9am-12pm at weekends.  Appointments are for routine and urgent matters and are booked by contacting this surgery during normal hours or via NHS111 outside of our normal hours.

There is also a GP at Orsett Minor Injuries Unit offering a walk in service and on day appointments for urgent medical issues only.  Open 11am-6pm Mon-Fri, 12-3pm Sat-Sun, appointments are booked directly with MIU (Tel 0300 300 1527) or walk in.  This service is temporary and may cease in the future.

Travel Notice - Malaria: It Only Takes One Bite!

Ensure you get advice on all the vaccinations required when you travel abroad. Take a look at the Travel Vaccinations page (under Clinics & Services) for further information. Remember to submit your travel forms at least 4 weeks prior to travelling.

Online Appointments Booking and Repeat Prescription Ordering

You can now have access to our appointments system online. By using this system you will be able to book an appointment with a GP up to four weeks in advance (no bookings for Same Day appointments).

New Patient Registration

registrationTo register you will need to be living in Chafford Hundred and to produce your medical card, together with your birth certificate/ passport plus a utility bill (dated within the last 3 months). In exceptional circumstances, we may be able to register you without production of a medical card but you will need to know your National Health Number. You will need to visit the surgery with your identification documents, at which time you will be booked a future appointment for a New Patient Health Check which are held on Wednesdays.  

Registration Times for Identification Paperwork to be processed

Mornings                 Monday-Friday           9.00am - 12.00pm

Afternoons              Monday - Wednesday  2.00pm - 5.00pm

                              Friday                        2.00pm - 5.00pm 

You will be registered with the group practice but you may wish to request to have a “preferred” GP of your choice. Please feel free to arrange this with the receptionist at your registration.

At your registration appointment, the nurse will discuss your medical history and undertake a BP and weight check. She will ask you to produce a urine sample (if you have not brought one with you, a bottle can be obtained from reception prior to your appointment).

If you are taking any regular medication, including contraception, you should bring the packets with you for the nurse to record. Please ensure that you book a routine appointment with the doctor to obtain your first prescription from this surgery.

Children under the age of five are not required to attend a new patient registration appointment. However, we will require some important information, such as immunisation history, and the receptionists will give you a form to complete to this effect.
