Notice to Close

For the purposes of Staff training this surgery, along with others in the Thurrock area, will be closed on the afternoon of  Tuesday 8th September 2020 .  Please note we shall be closed from 12.30pm.
During closure an Emergency Only service will be in operation and you are asked to telephone 01375 480000 where a contact number will be provided.
The surgery will open as normal on Wednesday 9th September 2020.
Whilst we hope you will agree that there is already a high standard of patient care in Thurrock we are committed to further improving Primary Care services.  Supporting protected learning time for GPs and Practice Staff will assist us in doing this.

IMPORTANT MESSAGE RE: CORONAVIRUS

We have put the following measures in place in order to help limit the spread of COVID-19 and provide a safer environment for patients and staff.

 

1    Attend the surgery only when you have been told to, ie for an appointment, to collect sample bottles or other information.  The majority of information can be emailed or posted to you.

2    Repeat prescriptions can be requested online (contact Reception to be set up for this).  Alternatively order through your regular pharmacy or drop your written request through our letterbox.  Please note you will no longer be able to write your request at the surgery.

3    Appointments must be requested by telephone or via the online triage tool (E-consult).  Please do not come to the surgery in person to book an appointment.  We are aiming to provide additional telephone support.

4    All appointments will be by telephone unless you are told otherwise.

5    If you are asked to come to the surgery for an appointment please wait in your car until your appointment time.  This is to limit the number of people in the waiting room at any one time.  

6    Do not bring friends or other family members with you unless essential, ie to accompany children or elderly etc.

7    Consultations are limited to a maximum of 10 minutes.

8    Please wear a mask or face covering whenever you enter the building.

9    We will be operating a one way system within the surgery.  Please enter through the usual main entrance, you will be guided to the appropriate exit.

FROM 13TH JUNE: Repeat prescription forms will no longer be available in the practice, this is in an effort to help to prevent cross contamination on pens and paper.  Please complete online request and return to surgery via postbox or email us at chafford.prescriptions@nhs.net

New Patient Registration

registrationTo register you will need to be living in Chafford Hundred and to produce your medical card, together with your birth certificate/ passport plus a utility bill (dated within the last 3 months). In exceptional circumstances, we may be able to register you without production of a medical card but you will need to know your National Health Number. You will need to visit the surgery with your identification documents, at which time you will be booked a future appointment for a New Patient Health Check which are held on Wednesdays.

You will be registered with the group practice but you may wish to request to have a “preferred” GP of your choice. Please feel free to arrange this with the receptionist at your registration.

At your registration appointment, the nurse will discuss your medical history and undertake a BP and weight check. She will ask you to produce a urine sample (if you have not brought one with you, a bottle can be obtained from reception prior to your appointment).

If you are taking any regular medication, including contraception, you should bring the packets with you for the nurse to record. Please ensure that you book a routine appointment with the doctor to obtain your first prescription from this surgery.

Children under the age of five are not required to attend a new patient registration appointment. However, we will require some important information, such as immunisation history, and the receptionists will give you a form to complete to this effect.
