Notice to Close

For the purposes of Staff training this surgery, along with others in the Thurrock area, will be closed on the afternoon of  Tuesday 11th August 2020 .  Please note we shall be closed from 12.30pm.
During closure an Emergency Only service will be in operation and you are asked to telephone 01375 480000 where a contact number will be provided.
The surgery will open as normal on Wednesday 12th August 2020.
Whilst we hope you will agree that there is already a high standard of patient care in Thurrock we are committed to further improving Primary Care services.  Supporting protected learning time for GPs and Practice Staff will assist us in doing this.

IMPORTANT MESSAGE RE: CORONAVIRUS

We have put the following measures in place in order to help limit the spread of COVID-19 and provide a safer environment for patients and staff.

 

1    Attend the surgery only when you have been told to, ie for an appointment, to collect sample bottles or other information.  The majority of information can be emailed or posted to you.

2    Repeat prescriptions can be requested online (contact Reception to be set up for this).  Alternatively order through your regular pharmacy or drop your written request through our letterbox.  Please note you will no longer be able to write your request at the surgery.

3    Appointments must be requested by telephone or via the online triage tool (DoctorLink).  Please do not come to the surgery in person to book an appointment.  We are aiming to provide additional telephone support.

4    All appointments will be by telephone unless you are told otherwise.

5    If you are asked to come to the surgery for an appointment please wait in your car until your appointment time.  This is to limit the number of people in the waiting room at any one time.  

6    Do not bring friends or other family members with you unless essential, ie to accompany children or elderly etc.

7    Consultations are limited to a maximum of 10 minutes.

8    Please wear a mask or face covering whenever you enter the building.

9    We will be operating a one way system within the surgery.  Please enter through the usual main entrance, you will be guided to the appropriate exit.

FROM 13TH JUNE: Repeat prescription forms will no longer be available in the practice, this is in an effort to help to prevent cross contamination on pens and paper.  Please complete online request and return to surgery via postbox or email us at chafford.prescriptions@nhs.net

Are You a Carer?

If you are please let us know - we would like to support you.

If you are agreeable, we will pass your details to the Carers Support Service, which is a countrywide organisation providing relevant information and advice, local support services, newsletter and telephone linkline for carers.

We will also refer you, with your permission, to have your needs assessed by Community Solutions.  A Carers Assessment is a chance to talk about your needs as a carer and the possible ways help could be given.  It can also look at the needs of the person you care for.  This could be done separately or together, depending on the situation.  There is no charge for an assessment.

If you would like us to refer you to the Carers Support Service and/or Community Solutions please complete the form above and indicate your wish in the  field.
