Notice to Close


For the purposes of Staff training this surgery, along with others in the Thurrock area, will be closed on the afternoon of  Tuesday 12th March 2024.  Please note we shall be closed from 12.30pm.
During closure an Emergency Only service will be in operation and you are asked to telephone 01375 480000 where a contact number will be provided.
The surgery will open as normal on Wednesday 13th March 2024

Prescriptions


Please allow THREE working days for repeat prescriptions to be processed, allow an extra day if your request has been sent after 11.30am.  Please bear this in mind when requesting your prescription, including if requesting via a pharmacy.  Thank you.

Pre-registration with the Practice

PreRegistrationYou can either attend the surgery to collect the registration forms or you can complete the pre-registration form online by completing the following steps carefully:

  1. Prepare a new email addressed to chafford.econsult@nhs.net with the subject 'Online Registration Request' for each new family member.
  2. Complete and attach the legal requirement form here:-  GMS1
  3. Then complete and attach the Registration form here:- Registration Form 
  4. Please then attach a copy of your Passport or Birth Certificate
  5. Lastly, attach a copy of a Utility Bill (dated within the last 3 Months) 
  6. Ensure all 4 documents are attached to each email for each individual. Please provide Copies as we are unable to return original documents.

Please send a separate email for each individual registration - even if you are 1 family.

Please DO NOT send your email until you have completed all the steps above - Your application will not be processed until you have provided ALL of these documents.  Please be aware that due to time constraints we are unable to chase missing data or documents, so it is well worth re-reading your forms to make sure nothing has been missed and they are completed correctly and to ensure that all documents have been attached to your email.
