IMPORTANT NOTICE REGARDING GP APPOINTMENTS
As a Practice we continue to regularly review the appointments system and make changes where necessary. At the start of the pandemic the appointments system was changed quite radically for our surgery (although many other surgeries were already operating a similar system). The Practice has decided to continue with the current way of booking appointments for the foreseeable future.
All GP appointments will be via the telephone unless you have been advised otherwise by the doctor, nurse or receptionist. Once you have spoken to the doctor they will book a face to face appointment for you as appropriate.
Please note that receptionists are not able to book face to face appointments with the doctor unless they have been specifically told to by the doctor.
Please remember that receptionists are only able to book an appointment for you if they are available and our appointments do get booked very quickly. It is not the fault of receptionists when all the appointments available are fully booked.
If you need non-urgent advice from a doctor or nurse please use the eConsult service (https://chaffordhundredmedicalcentre.webgp.com/
) in the first instance. As many of you already know, the telephone lines are extremely busy so we would appreciate it if the lines could be kept clear for more urgent medical needs and those who don't have access to the internet. Using this service also avoids you having to redial constantly and this can also be used for admin queries like requests for sick certificates and medication.
We do understand your frustrations at this very busy time - remember we are all patients too. We must all remember to be as patient as possible with each other and your co-operation is appreciated.
PLEASE WEAR YOUR MASK WHENEVER YOU COME TO THE SURGERY
The use of face masks is very important in reducing the risk of spreading the Coronavirus and all it's variants.
As a Practice we have made the decision to insist that ALL patients wear masks inside the surgery, including those who feel they are exempt. This includes while waiting in the waiting room and even if just popping in to drop off or collect some paperwork. If wearing a mask affects you, the doctor or nurse will let you know if you can remove your mask once you arrive in their consulting room.
We all need to do our bit to reduce the spread of the virus and especially be mindful that some patients coming to the surgery are already unwell. Staff members will also be wearing masks whenever they are in, or pass through, patient areas.
This decision has been made as a Practice, which includes admin and clinical staff, and we ask that you respect all staff at the surgery who may remind you to wear your mask.

Welcome to Chafford Hundred Medical Centre
At Chafford Hundred Medical Centre in Essex the doctors and nurses take pride in offering the highest standard of patient-centred healthcare.We run many clinics for chronic disease care and offer a wide variety of other medical services such as antenatal and postnatal care, minor surgery, childhood vaccinations and well-person check-ups.
This practice has been established for over 90 years and until July 1999 general medical services were provided at three sites in Grays, Purfleet and Chafford Hundred. It was at that time that the practice closed its surgery at Grays and moved into a purpose-built site in Chafford Hundred. In March 2003 an opportunity arose to relocate from Purfleet and this allowed us to concentrate all services from one development.
Appointments at other locations
Extra GP and Nurse appointments are available at a number of Hubs within Thurrock. Hubs are open 7pm-9pm every weekday and 9am-12pm at weekends. Appointments are for routine and urgent matters and are booked by contacting this surgery during normal hours or via NHS 111 outside of our normal hours.
Please note there is no longer a GP Service at Orsett Minor Injuries unit.
Online Appointments Booking and Repeat Prescription Ordering
You can now have access to our appointments system online. This Patient Online system requires you to have a user name and password in order to log on. To find out more and obtain a user name and password please speak to a member of the Reception Team. The service is not available for children under the age of 16.
By using this system you will be able to book a sameday telephone appointment with a GP. You will also have access to the Repeat Prescription ordering facility. This facility allows you to choose the medication from your medical record (repeat medication screen only) rather than you having to type into the current form on the website or an email. You will be able to see a list of your medications with dates when they were last issued to you. Utilising this service will enable you to access both appointments and prescriptions 24 hours a day.
The service also allows you to view your Summary Medical Record which contains details of your medication, allergies and sensitivities. If you already have access to online booking and ordering prescriptions you will need to reapply in order to see your Summary Medical Record.
