IMPORTANT MESSAGE RE: CORONAVIRUS

We have put the following measures in place in order to help limit the spread of COVID-19 and provide a safer environment for patients and staff.

 

1    Attend the surgery only when you have been told to, ie for an appointment, to collect sample bottles or other information.  The majority of information can be emailed or posted to you. Please do not attend with Registration documents - these can be emailed to us.

2    Repeat prescriptions can be requested online (contact Reception to be set up for this).  Alternatively order through your regular pharmacy or drop your written request through our letterbox.  Please note you will no longer be able to write your request at the surgery.

3    Appointments must be requested by telephone or via the online triage tool (E-consult).  Please do not come to the surgery in person to book an appointment.  We are aiming to provide additional telephone support.

4    All appointments will be by telephone unless you are told otherwise.

5    If you are asked to come to the surgery for an appointment please wait in your car until your appointment time.  This is to limit the number of people in the waiting room at any one time.  

6    Do not bring friends or other family members with you unless essential, ie to accompany children or elderly etc.

7    Consultations are limited to a maximum of 10 minutes.

8    Please wear a mask or face covering whenever you enter the building.

9    We will be operating a one way system within the surgery.  Please enter through the usual main entrance, you will be guided to the appropriate exit.

COVID-19 VACCINE: 

The public can really help the NHS to deliver our vaccination programmes effectively to those who need it most. Taking these simple steps will help us to help you.

  • The NHS will contact you when it’s the right time to come forward, so please don’t contact us to seek a vaccine before then. 
    If you are over the age of 70 (or know someone who is over 70) and have not yet had the vaccine, please contact the surgery to book an appointment.  We have now attempted to contact all our eligible patients in the first 4 cohorts (over 70 years old or Clinically Extremely Vulnerable) so if we have been unable to contact you (or someone you know) please let us know. 
    We have now started on cohort 6 (16-64 with underlying health conditions) and will contact you in due course to book your vaccine appointment.  We would appreciate your patience as we have over 700 patients in this group still to contact.
  • Patients in cohort 5 (over 65s) will be contacted directly by NHS England who are asking this group to book via the National Booking Service in order that surgeries can concentrate on patients with underlying conditions so we would encourage you to try that service first. 
  • Please do not phone the surgery unless you have been asked to - we will get to you. 
    Thank you for your patience.
  • Please act on your invitation when it arrives, and make sure you attend your appointments when you arrange them.
  • In the meantime, please continue to abide by all the social distancing and hand hygiene guidance, which will still save lives.

For information please note the categories of frontline workers who are eligible for the vaccine within the "Health & Social Care Workers" cohort.
Frontline healthcare staff:
Staff involved in direct patient care including - 
doctors, 
dentists, 
midwives and nurses, 
paramedics and ambulance staff, 
pharmacists, 
optometrists, 
occupational therapists, 
physiotherapists 
radiographers. 
Those working in independent, voluntary and non-standard healthcare settings such as hospices, and community-based mental health or addiction services.  
Staff working on the COVID vaccination programme, temporary staff, students, trainees and volunteers. 
Non-clinical staff in secondary or primary care/community healthcare settings including -
receptionists, 
ward clerks, 
porters and cleaners. 
Laboratory and pathology staff (including cleaners, porters, secretaries and receptionists 
Mortuary staff 
Frontline funeral operatives and mortuary technicians / embalmers 
Frontline social care workers includeing -
long-stay residential and nursing care homes 
social care staff directly involved in the care of their patients or clients 
Young people age 16-18 years, who are employed in, studying or in training for health and social care work should be offered vaccination alongside their colleagues if a suitable vaccine is available.
AT THE PRESENT TIME UNLESS YOU FALL INTO ONE OF THE ABOVE CATEGORIES WE ARE UNABLE TO OFFER THE VACCINE.  OUR PRACTICE MUST ABIDE BY NHS ENGLAND GUIDANCE.

Welcome to Chafford Hundred Medical Centre

.

At Chafford Hundred Medical Centre in Essex the doctors and nurses take pride in offering the highest standard of patient-centred healthcare.We run many clinics for chronic disease care and offer a wide variety of other medical services such as antenatal and postnatal care, minor surgery, childhood vaccinations and well-person check-ups.

This practice has been established for over 90 years and until July 1999 general medical services were provided at three sites in Grays, Purfleet and Chafford Hundred. It was at that time that the practice closed its surgery at Grays and moved into a purpose-built site in Chafford Hundred. In March 2003 an opportunity arose to relocate from Purfleet and this allowed us to concentrate all services from one development.

Appointments at other locations

Extra GP and Nurse appointments are available at a number of Hubs within Thurrock. Hubs are open 7pm-9pm every weekday and 9am-12pm at weekends. Appointments are for routine and urgent matters and are booked by contacting this surgery during normal hours or via NHS 111 outside of our normal hours.

Please note there is no longer a GP Service at Orsett Minor Injuries unit.

Online Appointments Booking and Repeat Prescription Ordering

You can now have access to our appointments system online. This Patient Online system requires you to have a user name and password in order to log on.  To find out more and obtain a user name and password please speak to a member of the Reception Team. The service is not available for children under the age of 16.

By using this system you will be able to book a sameday telephone appointment with a GP.  You will also have access to the Repeat Prescription ordering facility.  This facility allows you to choose the medication from your medical record (repeat medication screen only) rather than you having to type into the current form on the website or an email.  You will be able to see a list of your medications with dates when they were last issued to you.  Utilising this service will enable you to access both appointments and prescriptions 24 hours a day.

The service also allows you to view your Summary Medical Record which contains details of your medication, allergies and sensitivities.  If you already have access to online booking and ordering prescriptions you will need to reapply in order to see your Summary Medical Record.
